Matilda Medical Centre Terms and Conditions: - After the successful payment, client will receive a confirmation email from Welloft and client can book the appointment in the next working day. (Matilda Medical Centre email: mmc.central@matilda.org Tel: 2537 8500)- Client must present his / her identity document and the print-out of order confirmation email on the appointment day.- The purchase cannot be altered or refund after the confirmation of order.- All health screening test or examination is for the purpose of medical diagnosis or treatment- All packed items are fixed. In case there are any test items in the above programmes being declined or not conducted due to any reason, there will be no refund nor exchange for such items.- The programme is only available at the Matilda Medical Centre in Central.- This offer cannot be used in conjunction with other promotional offers or direct billing from insurance companies, and cannot be exchanged for cash or refund.- Additional costs will be incurred for additional examination, laboratory tests, medication, and the subsequent follow-up- The offer is not applicable for emergency health screening- Please make an appointment in advance.- Welloft and Matilda Medical Centre reserve the right of final interpretation.   Validity:The health check package is valid for six months (from the date of purchase) or it will be forfeited. Mainland or Overseas Clients (1) Face to face follow-up consultation within 3 months after assessment (2) Virtual follow-up consultation: pick up report by client or authorized person. Report also can be sent via email or post by mail with additional charges.
