After the successful payment, client will receive a confirmation email from Welloft and client will be contacted by Union Healthcheck Centre (H Zentre) within 2 working day for the confirmation of booking date and location. Client can also query your transaction in the next working day. (General Enquiry Hotline: 2682 2313)

Age
For customers aged 18 or above

Validity
The health check package is valid for six months (from the date of purchase) or it will be forfeited.

Report

Under normal circumstances, all medical reports will be ready around 7-10 working days (excluding Saturday, Sunday and public holiday). A delay in processing some requests due to the following reasons: 1. indicate the specific selective 2. Some items take more turnaround time for test.

A. Local Clients
(1) Face to face report explanation & self-pick up: in person or an authorized family member of friend
(2) Call report & self-pickup: in person or an authorized family member of friend
(3) Call report & post by ordinary mail. (Client should take the responsibility for the risk of mailing report.)

Time of self-collect:
Mondays to Friday (9 am to 5pm)
Saturday (9 am to 1 pm)|
Sunday and public holiday off

B. Mainland or Overseas Clients
(1) Face to face report explanation & self-pick up: in person or an authorized family member of friend
(2) Call report & self-pickup: in person or an authorized family member of friend
(3) Call report & post by mail with additional charges. (Client should take the responsibility for the risk of mailing report.)

Time of self-collect:
Mondays to Friday (9 am to 5pm)
Saturday (9 am to 1 pm)|
Sunday and public holiday off

Remarks:

1. Client must present his / her identity document and the print-out of order confirmation email on the appointment day.
2. The purchase cannot be altered or refund after the confirmation of order.
3. All health screening test or examination is for the purpose of medical diagnosis or treatment
4. An additional fee of $330 for extra report explanation section on request.
5. Unclaimed medical images would be discarded after 3 months. Please note an administration fee may be incurred for additional copies of medical record. Clients please proceed to Clinical Information Management Department in Union Hospital for the application for duplicated medical record/ medical report.
6. If individuals have special medical needs, the Union Healthcheck Centre (H Zentre) reserves the right to impose additional fees according to the circumstances.
7. DEXA bone density test is suitable for people over 40 years old (except for special needs)
8. Welloft and Union Healthcheck Centre (H Zentre) reserve the right of reserve the right of final interpretation.
